We are currently seeking a dedicated and detail-oriented individual to join our team as a Clinical Pharmacy Support. In this role, you will provide essential support to our clinical pharmacy team, assisting with various tasks to ensure the safe and effective delivery of pharmaceutical care to our patients. The Clinical Pharmacy Support plays a vital role in helping our pharmacy operate smoothly and efficiently, ultimately contributing to improved patient outcomes and satisfaction.

Responsibilities:

1. Medication Reconciliation: Assist with medication reconciliation processes, ensuring accurate and up-to-date medication lists for patients admitted to the hospital or transitioning between care settings. Collaborate with healthcare providers to resolve discrepancies and ensure continuity of care.

2. Patient Education: Provide medication counseling and education to patients and caregivers, explaining medication instructions, potential side effects, and adherence strategies. Answer patient questions and address concerns to promote medication safety and adherence.

3. Medication History Collection: Collect comprehensive medication histories from patients, caregivers, or electronic health records, including prescription medications, over-the-counter products, and herbal supplements. Document medication histories accurately and thoroughly to support clinical decision-making.

4. Prior Authorization Assistance: Assist with prior authorization processes for specialty medications, working with insurance companies, prescribers, and patients to obtain approval for necessary medications. Complete required documentation and follow up on pending authorizations in a timely manner.

5. Clinical Documentation: Maintain accurate and complete documentation of clinical interventions, medication therapy management activities, and patient interactions in electronic health records (EHR) or pharmacy management systems. Ensure compliance with documentation standards and regulatory requirements.

6. Medication Delivery Coordination: Coordinate medication delivery services for discharged patients, ensuring timely and accurate dispensing of medications to patients' homes or preferred pharmacy locations. Communicate with patients, caregivers, and pharmacy staff to facilitate seamless medication transitions.

7. Inventory Management: Assist with inventory management tasks, including medication ordering, stock replenishment, and expiration date monitoring. Maintain organized and efficient pharmacy storage areas, following proper storage and handling protocols for medications and supplies.

Requirements:

1. Education: High school diploma or equivalent required. Associate degree or completion of a pharmacy technician training program preferred.

2. Certification: Certification as a pharmacy technician (CPhT) preferred but not required. Willingness to pursue certification within [timeframe] of employment.

3. Experience: Previous experience in a pharmacy or healthcare setting preferred. Familiarity with pharmacy operations, medication terminology, and basic pharmacology concepts.
