RN Case Manager Position Overview

As a Hospice Field RN Case Manager, you’ll provide direct care to MJHS Hospice & Palliative Care patients through physical and psychosocial assessments in both community and facility settings. You will develop and implement nursing care plans tailored to each patient’s needs and document all visits electronically.

This role includes a rotating schedule with one weekend per month and backup On-Call responsibilities (2–3 times/month). Car and escort services are available for backup On-Call shifts.

RN Case Manager Responsibilities

• Conduct comprehensive patient assessments in the field

• Create and implement individualized nursing plans of care

• Document patient interactions and updates electronically

• Make ongoing care recommendations in collaboration with the care team

• Participate in On-Call rotation and weekend shifts as scheduled

RN Case Manager Qualifications

• Associate’s Degree or Diploma in Nursing required; BSN preferred

• Active State RN license

• Valid driver’s license

• 2+ years’ experience in med/surg, community health, or oncology nursing (preferred)
